When Value Isn’t Owned, Transformation Fails Every Time

Most large scale digital transformations do not fail because of a single mistake. They fail because, over time, the link between delivery and value weakens, and in many cases disappears altogether.

The programme progresses, governance is in place, delivery continues, and yet the outcomes that justified the investment are no longer actively measured or managed.

At that point, something important has already happened. Value is no longer owned.

What follows is familiar. Operations begin to strain, expected benefits fail to materialise, workarounds emerge, and confidence erodes. In the most visible cases, it ends in litigation.

This is not an exception. It is a pattern

The recent Zimmer Biomet SAP/Deloitte failure has been widely analysed, and for good reason. The scale of the impact, $172 million in litigation, billions in lost market value, operational disruption, and leadership fallout, places it firmly among the most visible transformation failures in recent years.

But it is not unique

Over the past two decades, similar patterns have surfaced repeatedly across industries and technologies:

  • Waste Management vs SAP
  • National Grid vs Wipro
  • Bridgestone vs IBM
  • MillerCoors vs HCL
  • Birmingham City Council vs Oracle
  • A growing number of NetSuite-related disputes

Different organisations, different vendors, different system integrators, but the same underlying trajectory. Large-scale transformation programmes begin with a compelling business case and clear executive intent, progress through structured delivery phases, and then, often shortly after go-live, transition from promise to disruption.

At that point, responsibility becomes contested. Vendors point to client-side decisions. Clients point to delivery failures. The underlying issue, the structural design of the programme itself, is rarely examined in any depth.

This is not a technology problem

It is tempting to attribute these outcomes to software limitations or execution missteps. In some cases, those factors are present. However, when examined collectively, these failures share a different set of characteristics, and they are not primarily technical.

What is consistently observed is that:

  • systems are technically live, but not operationally effective
  • processes exist, but are not consistently adopted in practice
  • data is available, but not trusted or actionable
  • business outcomes are expected, but not realised

These are not failures of capability. They are failures of operationalisation.

More specifically, they are failures to:

  • establish adoption as a measurable and managed discipline
  • validate value continuously, rather than relying on an initial business case
  • maintain independent visibility into whether the programme is delivering the outcomes it was intended to achieve

In other words, they are failures of value governance.

Where the current model breaks down

The structure of most large transformation programmes has remained broadly unchanged.

Technology vendors position outcomes such as efficiency, visibility, cost reduction, and improved control. System integrators deliver programmes through design, build, and deployment. Clients establish governance through steering committees, reporting, and oversight.

Each of these roles is necessary. But there is a gap between them

No party is explicitly accountable for ensuring that the promised business value is:

  • still valid as the programme evolves
  • being realised in practice
  • sustained after go-live

This gap is often obscured during delivery. Programmes appear controlled. Milestones are met. Progress is reported. Yet these signals are proxies. They indicate that work is being completed, not that value is being realised.

What is often overlooked is that these issues do not begin at go-live.

They are introduced much earlier, at the point where operating models are defined, business cases are constructed, and delivery structures are agreed. Assumptions are locked in. Incentives are set.

Accountability is distributed, often incompletely

From that point on, the programme follows a trajectory.

By the time problems become visible in operations, missed shipments, revenue impact, customer dissatisfaction, they are not new. They are the accumulated effect of decisions made at inception, compounded during delivery, and left uncorrected because there was no continuous visibility.

The absence of measurement

What distinguishes organisations that avoid these outcomes is not simply stronger governance, but better measurement. They do not rely solely on periodic reporting or delivery updates. Instead, they establish continuous visibility into:

  • how processes are actually being executed
  • where adoption is incomplete or inconsistent
  • whether expected benefits are materialising at an operational level

This allows them to detect divergence early, before it becomes visible in financial performance or customer impact. Without this level of visibility, organisations are forced to rely on interpretation rather than evidence. Issues are identified late, when correction is significantly more complex and costly.

What is required across the lifecycle

Addressing this gap does not require more governance forums or tighter contractual controls. It requires a set of capabilities that operate consistently across the full lifecycle of transformation.

1. At inception: establishing the right foundations

Before delivery begins, the focus must be on structural integrity. This includes:

  • validating the business case and expected value against operational reality
  • defining how value will be measured in practice, not just in theory
  • establishing clear ownership of outcomes, distinct from delivery responsibilities
  • designing an operating model that separates execution from independent validation

At this stage, relatively small decisions have a disproportionate impact.

If value is not clearly defined and owned here, it becomes difficult to enforce later.

2. During delivery: maintaining visibility and control

As the programme progresses, the emphasis shifts to continuous validation and alignment.

This requires:

  • tracking adoption readiness alongside technical progress
  • identifying divergence between delivery signals and operational reality
  • monitoring scope, cost, and timeline changes in the context of value
  • providing independent insight into whether the programme remains on track

Most programmes do not fail abruptly. They drift.

Without continuous visibility, that drift goes unchallenged until it becomes embedded.

3. Post go-live: ensuring value is realised and sustained

The most critical phase is often the least actively managed. After go-live, attention typically shifts away from the programme. Yet this is where value is either realised or lost. At this stage, organisations need to:

  • measure how processes are actually being executed in practice
  • identify adoption gaps and behavioural resistance
  • link system usage directly to business outcomes
  • optimise continuously based on real-world performance

Without this, programmes can be technically complete while commercially underperforming.

Recovery is not an exception, it is part of the model

An increasing number of organisations are not starting from a clean slate. They are dealing with programmes that are already live, but not delivering the expected value. In these situations, the challenge is not implementation. It is recovery. This requires a different starting point:

  • establishing a clear view of how the system is actually being used
  • identifying where processes have broken down or diverged
  • reconnecting system behaviour to business outcomes
  • stabilising operations before attempting further transformation

The same capabilities that prevent failure, adoption visibility, value measurement, independent validation, are equally critical in recovery. The difference is that they are applied retrospectively, often under greater operational pressure.

Recovery, therefore, is not a separate discipline. It is the same model, applied later in the lifecycle.

A model that works for clients, integrators, and vendors

What is emerging is not a rejection of the current model, but an evolution of it.

For clients, it introduces continuous confidence that transformation is delivering real outcomes, not just progressing through milestones.

For system integrators, it creates a clearer separation of roles, reducing the inherent tension between delivery and validation, while opening the opportunity for longer-term, outcome-based engagement.

For technology vendors, it increases the likelihood that their platforms deliver measurable value in practice, protecting both reputation and long-term growth.

In this model, success is no longer inferred. It is observable.

A shift already underway

Better planning helps. Stronger governance helps. More experienced partners help. But none of these, on their own, address the underlying issue. What is required is a structural change, one that ensures value is:

  • defined at inception
  • validated during delivery
  • measured after go-live
  • and actively recovered where necessary

Without that, organisations will continue to rely on delivery as a proxy for success, and as recent history shows, that is not a reliable measure.

Digital transformation does not fail at go-live.

It fails the moment value is no longer actively designed, measured, and owned across the lifecycle.
